WHERE TO SECURE

1.    Original copy of legibly filled-up Application form (AL-01), with condition below;

1.1.   Indicated therein are valid Security Officer (SO)/Security Consultant (SC) license of the Licensee and Staff.

2.    Original copy of the previous License to Operate;

3.    Biographical Data of the New Licensee with conditions below;

3.1.   College Diploma of New Licensee (authenticated by the school registrar of the same school) and Transcript of Records of Licensee (Original copy issued by school registrar of the same school);

3.2.   NBI Clearance of Licensee (No Derogatory result and issued not later than three (3) months from the date of filing LTO application) (Original copy); and

3.3.   Original/Authenticated copy of License to Exercise Security Profession (LESP) [Security Officer (SO) license] of New Licensee.

4.    Original copy of RESUME for Directors and Partners, with conditions below;

4.1.   For Licensee – attach College Diploma and Transcript of Records (TOR);

4.2.   For Staffs and Directors – RESUME or Personal data sheet only.

5.    Registration of Certificate from DTI or SEC with conditions below; and

1.1.   For Single Proprietorship – Certificate of Cancellation issued by the DTI; and

1.2.   For Corporation – Updated General Information Sheet (GIS) issued by the Security and Exchange Commission (SEC) with an Amended Articles of Incorporators.
